Pipe Cap Failure Blamed for UNI Worker's Death

CEDAR FALLS, Iowa (KCRG) -- A cap failure on a steam pipe caused a University of Northern Iowa maintenance worker's death last month. That's according to the Waterloo-Cedar Falls Courier who got a report from HBK Engineering.

UNI hired HBK to investigate the incident. Kevin Bley, 61, died from injuries while in a steam tunnel in one of the university's dining halls on September 10.

HBK's report says there was a steam release from a two inch pipe that had a "sudden, catastrophic failure." Bley had worked for UNI for a decade.
